How to Report a Forgery to TRS Recovery Services, Inc.

If you need to notify TRS that you have been a victim of forgery or identify theft, you should complete the form below and return it to TRS. Please allow 30 days for TRS to process and respond to your dispute. We also ask that you include copies of any other documentation you have that is relevant to the investigation and substantiates your claim such as:

  • A copy of a filed police report;
  • A completed FTC Identity Theft Report Affidavit available at;
  • A completed bank affidavit or declaration, or
  • Copies of receipts that reflect the fraud
